A fugitive wanted by the FBI over an alleged USD 3.7 billion Medicare fraud scheme described as one of the biggest healthcare fraud cases in US history has been arrested in Turkey and flown back to America to face justice.

The arrest came after an international operation involving Turkish authorities and US law enforcement, with Ibrahim Khaldoon Hilmi transferred from Turkey to South Florida after being detained overseas.
US authorities allege that Hilmi played a key role in one of the largest Medicare fraud cases in American history.
According to the FBI, the suspect had been on the run since May 2025 after fleeing the United States while under investigation.
Federal investigators claim the scheme targeted Medicare, the US government health insurance programme, and involved fraudulent billing and reimbursement claims worth an estimated USD 3.7 billion (GBP 2.8 billion).

The FBI said Hilmi was located and detained by Turkish authorities before agents travelled to Turkey to take custody of him.
Photographs released by the FBI on 22nd June showed Hilmi departing Turkey during daylight hours before arriving in South Florida last Friday.
FBI Director Kash Patel said: “Ibrahim Khaldoon Hilmi is charged with one of the biggest Medicare scams in history – allegedly orchestrating a massive USD 3.7 billion scheme to defraud Medicare.
“He’s been on the run since May 2025 – but we got him.
“Thanks to outstanding work from FBI Miami, the Justice Department and our partners in Turkey, Hilmi was apprehended overseas and after a foreign transfer of custody, he is back in the US to face justice.”

Patel described the arrest as a major victory in the FBI’s efforts to track down fugitives accused of stealing public funds.
The FBI said the case was investigated jointly by its Miami field office and the US Department of Health and Human Services Office of Inspector General.
Prosecutors claim the fraud operation exploited Medicare’s reimbursement system to obtain huge sums of taxpayer money.
Hilmi’s group allegedly purchased more than 30 legitimate U.S. companies and transformed them into shell fronts.
The fronts were explicitly used to bill Medicare for Durable Medical Equipment (DME), primarily focusing on billing millions of dollars for catheters that were never actually provided to patients.
Once Medicare paid out the claims, the network laundered the proceeds globally, moving the stolen taxpayer money directly into bank accounts across China, Singapore, and Turkey.

The operational infrastructure of the fraud ring illegally manipulated and involved over 7,000 legitimate physicians and healthcare businesses.
Federal investigators said they stopped the alleged scam in its tracks by freezing accounts before billions of dollars could be paid out.
Authorities claimed just USD 41 million (GBP 31 million) of the gross USD 4.45 billion (GBP 3.36 billion) in total healthcare claims submitted by the network was released before the operation was shut down.
The remaining USD 4.41 billion (GBP 3.33 billion) was frozen and secured for return to Medicare and private insurers, while investigators noted that of those gross claims, USD 1 billion (GBP 757 million) had been targeted at private insurance companies.
Following his return to the United States, Hilmi is expected to appear before a judge in Miami, where the case is being prosecuted.
Director Patel specified that the takedown was a flagship victory for the newly established White House War on Fraudsters Task Force, which is actively chaired by VP JD Vance.
VP JD Vance said authorities remained committed to pursuing suspects accused of defrauding American taxpayers regardless of where they fled.
The FBI added that its efforts to track fugitives overseas would continue and said its determination to bring fraud suspects to justice “will not waver”.
